September 27, 2003 Event Reservation Form

To pay to attend the event, please fill out the form and then click the submit button to send the form via e-mail, OR, print the form and snail-mail it to the PO Box as indicated. You may pay for reservations in person at Parkland National Auto Parts Monday through Friday 8 am to 5 pm (See Michael Blair). Payment may be by check (Payable to "Donation Account for Michael Blair"), OR cash (if paid in person). Credit card payments will be processed on the night of the event--(for event reservations and for payment of auction items). Your event receipt will serve as your event ticket and it will be mailed to you if your payment is received prior to September 22nd, 2003, otherwise, your receipt may be picked up at the "CHECK IN" area on the day of the event (after 4:30 p.m.)
